• A telephone number at which you can be reached to confirm your reservation - THIS IS VERY IMPORTANT, OTHERWISE WE CANNOT CONFIRM YOUR RESERVATION
• Name you would like the reservation to be held under
• Number of people in your party
• Any special requests or needs for your party
